Forms Issued by the Department of Social Security

Form H.C.1:

i) a means test to ascertain if help is available to meet dental charges in cases where exemption is not automatic

ii) sent by patient to central office in Newcastle

iii) result may be partial or complete exemption of charges.

Form A.G.5

i) form issued to patients from local Social Security Office only.

ii) issued to patients who have already paid NHS charges but may be found subsequently to be exempt or partially exempt.

Certificate A.G.2

– issued by the DOH certifying that the holder is completely free from all forms of charges for NHS treatment eg. prescriptions etc.

Certificate A.G.3

i) issued in cases where patient is partially exempt

ii) dentist is responsible if this certificate is presented to check its validity before acceptance. Both Cert. A.G.2 and A.G.3 are valid for 6 months.

Exemption from NHS Dental Charges:

– pregnancy

– baby born in last twelve months

– under 18

– under 19 in full-time education.

Full/Part Remission:

Patient or partner – obtaining Income Support or Family Credit

– has a DSS Certificate for help with NHS charges
